According to the Belastingdienst BTW brochure: a sequential number, your name, address and FIN, the date, the goods or services, the amounts excluding and including BTW, the BTW rate, and a note when an exemption applies.

Does the amount in words follow the official spelling?

Yes. Dutch amounts follow the Taalunie rules (for example 'duizend tweehonderdvijftig'), with 'één' written with accents to avoid confusion.
